FEES FOR COMPLETED FORMS, LETTERS AND MEDICAL RECORDS

Applicable Fee for all forms and letters:

       $25 – EACH set of forms

Fee for Medical Records:

       $1.00 per page (up to 25 pages)

       $.50 per page (page 26+)

       Cost of postage if not picked up by patient

***PREPAYMENT REQUIRED FOR ALL FORMS, LETTERS AND RECORDS BEING MAILED OR SENT ON YOUR BEHALF FROM OUR OFFICE.

*** PAYMENT MAY BE MADE AT PICK UP FOR ALL RECORDS, LETTERS AND FORMS BEING PICKED UP BY PATIENT/PATIENT REPRESENTATIVE
