Our Office Policies

We look forward to seeing you on your scheduled appointment date. If you are a new patient, information is being sent to you in an effort to streamline the check-in process and answer any questions you may have regarding your visit. We will be contacting you a few days prior to your appointment to confirm the appointment date and time, pre-register your demographic information. When you check-in we will need to copy your insurance card(s) in order to bill your insurance company.

Mission Statement

The mission of the Dysautonomia-MVP Center is to provide quality health services and facilities for patients with Dysautonomia, to promote wellness and improve quality of life focusing on correct diagnosis and patient education, establishing a treatment plan that is specific for each patient,  following each patient through their exacerbations and remissions of Dysautonomia, to relieve suffering, and to restore each patient to the best health possible as swiftly, safely, and humanely as it can be done, consistent with the best service we can give at the highest value for all concerned.

Billing

The Dysautonomia-MVP Center accepts cash and personal checks as well as the following credit cards:

  • MasterCard
  • Discover

 

All co-payments and deductibles are due at the time of service.  If you have questions about payment for your services or out of pocket cost, please contact our billing office, [email protected].

Appointments/Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require advance notice. In such a case, please call our office and reschedule your appointment as soon as possible.  Missed appointments may result in running out of medication which ultimately could worsen symptoms.

New Patients:
In the event that you need to cancel your appointment, kindly provide 48 hours notice so you are not subject to a $100 Cancellation Charge. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.

Returning Patients:
In the event that you need to cancel your appointment, kindly provide 48 hours notice so you are not subject to a $25 Cancellation Charge. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.
